Question for all of you who bill Tricare South:
They have started requiring chart notes for almost everything we bill.  In particular, we are having a terrible time with home visits. These are postpartum home visits, and coded as such, but they are denied with reason: "send clinical information, including homebound status and patient goals". The denials are a new occurance and these visits are getting lost in the black abyss of home health care. Attempts at explaining over the phone are futile, and sending chart notes with detailed explanations has only resulted in limited success.  I'm not sure what happened, but Tricare South has become a bear to work with lately. We do not have this problem with TriWest or Tricare North (and have pointed this out to Tricare South, but they merely sniffed their indifference).

Any suggestions as to how to avoid the home healthcare abyss?

I have not had this problem in the mental health side as of yet.    I hope this is just a brief issue for you.  I will continue to pay close attention to see if they start to do this across the board.

I to have the same problem with one of our therapits on her inpatient chrages at the hopsital they have requsted medical records.

At least it's not just us, but it is still very annoying.
