With the new E/M changes, if you bill 99205 based on Medical decision making, can you still bill 99358 for additional non-face to face time.  There's usually about 45 minutes spent reviewing records, labs, pathology and consulting with referring doctor, dictating the note after the appointment.

With the new changes, time spent after the visit are to be counted with the time spent with the patient.  So if the total time spent exceeds the 60 minutes typically spent, then you could append the 99358.

Thank you Michele!  The new time range for 99205 is 60 to 74 minutes.  But, If I am not basing the visit on time, I am basing it on Medical Decision making, can I still bill 99358?  Our new visits are scheduled for 75 minutes, but there is usually a lot of work before and after a visit.

As long as the notes/chart support the 99205 and the add on they can be billed.
