We had a patient come in the office in the morning sick with cough congestion and bilateral ear pain. She also had two moles that she was very concerned about. Due to time restraints she was asked to come back in the afternoon for the procedure. What is your suggestion for billing both?

I would recommend billing the E&M code for the office visit with the diagnoses cough, ear infection, etc that related to the cold/cough. Then I would bill the removal of the moles with the appropriate diagnoses with a 59 modifier showing separate service. I don't think it is necessary to indicate separate time of day. If it is denied, then I would certainly appeal, and include in the appeal that the visits were two separate times.
