Mistake was made when billing for a visit.  units where entered as 95 instead of 1.  I went to united health care online and submitted a appeal with documentation.  Did I do this right or should I have done it differently.  The lady who normally handles this kind of stuff is no longer with us and trained no one before she left

In this situation, I would have just submitted a corrected claim with the correct amount of units, not an actual appeal with treatment notes.

I think it should be fine that you did an appeal.  Have you checked it to see if they are processing it?  Or you can call to ask, but if I do an adjustment I do it thru the website.  I believe there is a way to submit an adjustment request.

In the future, if you do need to correct a claim, you can do so by correcting it and resubmitting it electronically with the correct frequency code if your software requires that, or you can just correct and resubmit, or if the claim goes on paper (very rare circumstance), you would correct, reprint, and you can write Corrected Claim on top of CMS form. Some insurances will also allow you to correct through their portals, or Availity or Navinet if the insurance uses those websites.
