Hello, we recently closed our Chiropractic office and the Dr. is now sharing space at another office (due to Pandemic).

I changed the address of the office  to our home address, where I do the all the billing.  All of our records have been changed to reflect the home address for any correspondence relating to the business.  He is seeing our patients at the new location, but again our Tax ID and all information relating to the office was changed to our Home address.  Our Chiro billing software also shows the home address in Box 33, and I change the facility address (Box 33) accordingly.

I recently spoke to an Insurance rep from (Avmed) here in Miami, FL – and was told that our claims would be “rejected” because the Facility Box (32) is different from Box 33 (Billing Provider)?

I explained that this not unusual and in fact done frequently? Many Doctor's see their patient's at Nursing Home Facilities, or Hospitals and they bill from their respective address ?  Has anyone had an issue with this?  Not sure how to proceed?

It is common for the facility address (box 32) and the billing address (box 33) to be different.  However, you didn't mention in your post if you updated the practice location to the new office that the dr is sharing. 

However, Avmed is indicating that if box 32 and 33 are different they will deny, which I haven't heard of before.  I would ask them how they want you to submit?  Using the billing address for both even though the patient wasn't seen there?

Yes, we did change our business address to our Home address where we will be doing the billing.  I did add the Facility address as a 2nd address on CAQH

It seems Avmed wants both addresses to be the same, and we do not why if this is not uncommon on claim forms ?

I have submitted many claims with different addresses in boxes 32 & 33.  We do it everyday.  This must be an Avmed thing.  I would contact them to ask if they want you to use the billing address in both.
