I had a good physician, and she always put 1,2,3,4 like on the claim form.

The physician should determine which dx's go with which procedures.

As the ladies have mentioned, the MD should be linking the CPT's with the DX codes

so if the provider does not show this on info sent to you like the encounter sheet you can just request this?

Yes, you should ask them to please indicate the order of dx's, and which dx's for which procedures.

Hi Margie,
If it's something simple, like internal medicine billing, you can usually determine which ICD-9 is linked to which CPT/HCPCS code! But, you are speaking of DME, correct? I suppose Pay My Claims is the expert on that!

Ha ha......It works just as simple with DME. You know that OA isn't a reason to obtain a high profile pressure relieving cushion so you choose decub ulcers for that.
