CPT code 99457 Remote patient monitoring

Does anyone have any experience with remote patient monitoring? We started this program a couple of months ago and the first claims went thru with no problems and were all paid. The next month of monitoring I had several denials for this code stating that the payment adjusted because the payer deems the information submitted does not support this many/frequency of services. I checked for any time limit restraints on Medicare's site and it just states that is based on calendar month. Does that mean that it has to be exactly a month from the date I billed this the last time?

We bill chronic care management and it had the same guidelines,but as long as it is in the next month we get paid. We have not had to wait for it to be exactly one month apart.

I haven't billed for this so I am not sure.   Have you tried to contact them to ask if it goes by 30 days or the calendar month?

What type of practice you run? Is it PCP or Behavioral health?
