Hello,
The provider that I'm working with doesn't have a fee slip with the $ amount that he charges the insurance. Where can I find a generic one that I can input into the medical billing software? Also how would I be able to obtain the insurances allowable amount? Would I have to call and get the fee slip schedule from them? Please help...

Some softwares have the capability of doing superbills, that's probably what you want? Fee schedules come from the carriers your provider is contracted with, check the contracts or get in touch with the carriers for their fee schedule.

For the billed amount to charge insurance, we will need to know more, such as what the specialty of the doctor is, his location, etc. But if it would help, we normally go by adding 200-300% of the Medicare fee schedule for the pay locality of that provider.
The allowable amount would normally depend on the contract your provider signed/accepted with the insurance/payer.

That's great. I have 2 providers with that specialty. One is a PCP and another a Hospitalist.
