Hello,
We have a private company that is providing phlebotomy services to an APRN in Connecticut, the blood samples are being sent to Colorado for  right now. How can we bill for these services?

I am only aware of being able to bill the 36415.  You may also be able to bill for travel fees using P9603 or P9604 depending on what you are actually doing.
