I need help with diagnosis coding again.
Mixed benign proliferative breast lesions of the right breast s/p right breast segmental resection.
Lesions include:
1) Complex papilloma with atypia
2) Atypical ductal hyperplasia
3) Flat epithelial atypia
The above are in the presence of micro calcifications which increase the above lesions prediction for subsequent diagnosis of breast cancer.

No Im not but sometimes they dont code it, then i will code it.

Not a good idea. Give it back to them, you can be held just as libel as the provider if there is a problem.

Wow Ladies/Gents...this kinda hit home for me...
Since I've started, they've just kept giving me diagnosis names not codes.
I had to look up codes for each new diagnosis that I wasn't aware of before.
Since start of 2007! :o

You need to understand that regardless of your employment or company contract with the provider that you can be held JUST as accountable as the provider. You have to insist they do the coding or hire an in-house certified coder. Anything you bill out, regardless of your status (employee, billing company) is your responsibility to know.
