If "Dr Need Help" is both the ordering physician and rendering physician.
 
Would I include Dr Need Help info in box 17, 17b(npi) as well as (NPI) in box 24 J

If the service requires an ordering physician, yes.

Does PT services billed by a ARNP or MD requires an ordering physician info in box 17, 17A

what's the difference in Physicial therapy and Therapy/modaility services.   The codes are the same when billed by a PT or a chiropractor.. I've been researching and I'm still a little confused.
Is it consider therapy/modality when performed by a chiropractor and  Physicial therapy (pt visit) when done by a physicial therapist, ARNP or MD.

Medicare requires an ordering physician, other insurance carriers may vary.  Therapy services are therapy services.  When performed by a chiro most refer to them as modalities, but they are still therapy services.
