Physical Therapy services at Group Home

Hello,
I am new to Physical therapy billing. I have few questions. I will truly appreciate any kind of help.
I am billing for a physical therapist who does house calls. He is currently seeing patients at a Group Home. For Box 32 on the 1500 form should I put the address of the Group Home and license number. Also, for box 17, should I enter the referring physician name and NPI?

Are you billing with POS 12 or 14?  If billing with POS 12 I would use the group home address but have it under the patient's name with no NPI.  The group home license number is not necessary.  Not all group homes will have an NPI, so that is why I'm asking.  If they do have an NPI then I would use it.

Yes the referring physician & NPI should go in box 17.

Thank you for your reply Michele. I wasn't sure if I should use POS 12 or 14. Since, this group home doesn't have NPI listed and has a license number. I will bill it as a house call with POS 12. I really appreciate your help  :)
