We have a new COTA at our facility.  I know patients she is treating must be supervised by an OTR and the OTR has to do the evaluation and get authorization under the OTR's NPI and TPI (TX Provider ID).  And we bill under the OTR's NPI along with a modifier (UB) indicating the patient was treated by the COTA (because COTA cannot get a TPI).  This is what I've read in Medicaid manual.  So, I hope that part is correct. 
My questions is, I know the OTR has to do supervisory visits, but I would like to know how often AND can we bill for the supervisory visits under the OT NPI?   The reimbursement is lower for COTA visits, but the supervisory visit would be paid at the OT rate.

What state are you in?   This information should be in the manual.  There is no national guideline for this.

In Texas.  I have looked at the manual.  I only see info about billing for COTA but not about whether or not we can bill for the supervisory visit.

Have you tried contacting a customer service rep to see if they can point you to where the information can be found?  That is what I would try next.
