When a patient's plan of care is recertified or when they are discharged is there a HCPCS code for that? And would private insurance be billed the same as medicare, as far as the coding?

Physician re-certification for medicare-covered home health services under a home health plan of care (patient not present), including contacts with home health agency and review of reports of patient status required by physicians to affirm the initial implementation of the plan of care that meets patient's needs, per re-certification period
I don't see anything for the discharge.

I have another question regarding Home Health certs/recerts. When I bill what date should I be using? Example the start of care is 5/17/09 and the MD signed off 6/25. Should I use the 5/17 or 6/25? It's documented in the chart as 6/25 review. If I use 6/25 and the MD signs the next order as soon as it's received then I would be billing less than 60 days apart. Can someone please help clarify this. Also does the start date need to go on the claim form? We are just starting to bill for these services.

Are you billing on a CMS form or a UB04?

The start date should go in box 14. The dates that you are currently billing for should go in 24a. If you had a recent order from the dr that is after the start date I would put that in box 19.
