Hi Michelle
I am doing billing for a private PT. How do I bill with KX modifier in CMS 1500 form? Could you please give me some examples?

thats new for me, I only thought the KX modifier was used in DME

the first article instructions termed in 2007, its interesting since I only associated the KX with DME. It appears to be relative to the cap on PT ...

this info would help.  PS. is there a code that I have to include in the CMS 1500 form to indicate I am submitting the bill with KX modifier? what are the codes and which line should I put it under?? 24D???

Yes 24D is for the modifiers, including KX.

Yeah, 2007 was the newest I could locate on the subject that had more detail. It was more a point of reference.  :)

You put GP as the first modifer (as you normally do), and KX as modifier 2.  HTH
