: simple custom report for lytec

I need to make a list of every service provided by my office for the last 6 months. This list needs to consist of:
Chart identifier
patient age on date of service
Gender
Date of Service
Primary icd 10 code
CPT code

I tried to create a custom report using
Reports->custom reports

but cannot find the proper fields to use.
In addition, I cannot preview the report.
And finally, I cannot select a date of service range.

Is there a simple 'how to' somewhere of how to do this?

I have created many custom reports in Lytec but what you are looking to do is a bit too complex for the report functions you have to work with.  It's not that it can't be done, but it really has to be done by someone at Lytec.  We had to have our reseller write us a similar report once.  It worked great.  Cost a little (few hundred dollars) but it was well worth it.  If you want I can give you his name.
