I moved to another state and I am a registered Medical Billing Service with Medicare. I was wondering if any one knows how I can update my address with Medicare. I called them and they suggested the 855B form. When reviewing this however I don't think it's the correct form. Does anyone have any insight on this?

Does Medicare send your billing service payments made out to you directly? If not, I don't believe you have to change your address with them. If they do, it would be either form 855-R for reassignment of benefits to your company, or 855-B for other organizations.

Enrolled Medicare physicians are required to report billing agencies and/or any changes made go on the 855B

1, 2B1, 3, 8 (complete only those sections that are changing), 13, and either 15 (if you are the authorized official) or 16 (if you are the delegated official), and Section
6 for the signer if that authorized or delegated official has not been established for this provider.

Billing services don't update address changes directly to Medicare but the providers that you bill for must report any changes. (Which you might want to do as their billing service since it's your information that changed.) So depending on whether they are individuals or groups will determine the correct form. Any group would use the CMS 855B that Linda mentioned. Individual providers would use the CMS 855I which can be found at https://www.cms.gov/Medicare/CMS-Forms/CMS-Forms/Downloads/cms855i.pdf You would check section 1A "changing your Medicare information" and then section 1B as "billing agency information". Then you only need to complete sections 1, 2A, 3, 8 (complete only those sections that are changing), 13 and 15.
