: Medicare Eye Exams- Medical diagnoses

I am trying to find out if Medicare has a specific list of medical diagnosis codes that would be covered for medically related eye exams (non routine). I have looked through CMS LCD's and cannot find anything like that.  Basically, I am trying to understand if Medicare would cover any medical diagnosis for an eye exam that isn't routine.

Medical eye exams are covered for any dx that is not considered routine.  Testing such as 92250, 92225, and 92083, etc does have specific conditions that must be indicated for those to be covered.  They have lists of covered dxs for those codes, but eye exams are anything other than routine.
