One of my providers is looking for some type of software to just store patient demographics and print encounter forms with the patient information on them. They don't want anything else, they have me to do their billing and they schedule their appointments in writing in an appointment book. They are very old fashioned and like it that way. Does anyone have any suggestions for this? Any help would be appreciated. Thank you.

Appointment books should always still be kept even with scheduling software because if ever an audit that's first thing they want is the actual appointment book :)

I would imagine they could use any PM software to do what they want, though they know they will be doing double entry? Problem with that is that if they are entering charges onto the encounters it could be construed as having two sets of books..which is a no-no. I have clients that do this but they login to the PM software/server and do it right from there. Do they have access to your software?
