Ulcer debridement

I have an E&M, 11721, and 11042 to bill; all on the same date of service.  However, for 11042, do I use modifier 25 or 59, or nothing at all?

If you read the description for modifier 25, you will see that it is ONLY to be appended to E/M codes, and never any other type of code. So you can rule out using the 25 modifier on the 11042. Depending on the insurance, the 59 modifier will have to go on the 11042, the 11721, or both.
