there is a cpt code for filling medical forms at the office that i can use

Are you talking about completing forms for the patient such as disability forms? If so, I'm not aware of a CPT code for that since it is not billable to insurance companies. If anyone else knows something on this, please share.

99080 is the code to fill out forms. However, insurance companies don't pay this. All of my providers have a set fee that they charge the patient. They all vary. Some just want the pt to drop off the forms and charge $20. I have one doc that requires an appt be made and charges a standard office visit fee, paid up front. I hope this helps.

Thanks, I totally missed the 99080 code. None of my providers bill it since it is not usually covered. Good info.
