UHC Medicare plan denied cpt 37241 stating not covered by Medicare. Where do I find if covered by Medicare.

It may be due to the dx code. There should be other information on the denial such as not a covered procedure under Medicare. It may even list an LCD number. Without additional information it's hard to help.
