: Chiropractic Modifier/Advent Health Insurance

Hello!
I need some help with my Advent Health Insurance claims.. They keep denying 98940 and 98941 stating "Procedure Is Not In Accordance With Medicare Lcd Policy". I've tried AT modifier and they are still denying with the same reason.
Any suggestions?

Are your dx's covered by Medicare LCD?
