Whats a medical necessity note?  A letter from the provider showing medical necessity for the test or procedure?  This is my bad ay when things dont click easily

I think, you have question and an anwer in your query. Normally the medical records would be appealed/submitted to explain the meidcal necessity of the service.

To answer you question, Medical necessity notes - can be medical records/ER reports/Lab notes/xray reports[based on the service]. I think, you got the answer.
