another question: when billing mri on PIP cases and to insurances, do you have to put the referring physician info in box 17:

example doctor B referred the patient to doctor A to have a mri done...

do I bill with doctor B  name in box 17 and npi# in box 17B, then doctor a npi in box 24j and 33a

Referring dr & npi in box 17
Billing dr/group in 33
rendering provider individual NPI in box 24J
