: Holter monitors

A holter monitor was placed on a patient on 5-1-12 through hospital outpatient department.  My physician reads the holter monitor on 5-4-12.  What date would I use to bill out for the PHYSICIAN READING of the holter monitor - 5-1-12 the date holter was placed or 5-4-12 the date the reading actually took place?

if billing for the reading, the date the reading was done,  I believe.  I would go with the 5/4/12.

I have been using the date of the actual reading (ie 5-4-12) but I was second guessing myself.

You would use the date that the holter monitor was read. That way it is the same date as documented in the medical record.
