Can the CPT description be used in field #43 instead of the Rev code description. 
Should each CPT code be listed in field #44? 
How does the facility bill for bilateral, do I use modifier 50 or use quantity "2"?
Should Procedure codes be used in line #74?

Can the CPT description be used in field #43 instead of the Rev code description.    Whatever best describes the service indicated for that service line should be indicated
Should each CPT code be listed in field #44?      If there is an appropriate CPT code it should be used
How does the facility bill for bilateral, do I use modifier 50 or use quantity "2"?     If you are billing facility fees the bilateral shouldn't be a consideration.  The facility charge should be the same whether the surgeon does bilateral or unilateral.  The bilateral would be indicated on the professional fees only
Should Procedure codes be used in line #74?    box 74 should be the cpt code for the main procedure that was performed
