I have billed Medicare for 99397 and 99495. They paid on 99495 but not the other. When I submitted the claim I did not include a modifier. How do I get Medicare to pay on both? I'm not sure what modifier to use.
TIA

You cannot use the E&M code 99397. Medicare has a specific code for yearly exams, G0439.
