I’m new to this forum so forgive me if I do something wrong, I need help with this denial I’m receiving from a MCO.
Here’s the scenario: O/P - on a UB-04 form -( 7/25-7/27 ) place of service 22;
Denied for no apl code for dos or missing rev-proc combo for er obs psych.
Here’s some of the codes on claim: 258, 96365, 82565, 80202.
I’m having a hard time understand what is needed for APL CODE. Is it both rev code and cpt/hcpcs codes? I can only find the 96365 code that is listed on the APL LIST. Needed help to clean up these claims and find the issue. Thanks so much!
