NPI/EIN for DBA

My doctor is in the process of a business name change/expansion, however his tax ID is staying the same. We are justing using a new DBA name. When adding a new provider to our group, we use our tax ID and our legal name under that, NOT our DBA? Is that correct? Or can we add our DBA to our cms1500 as well?

Your NPI, Tax ID have to match EXACTLY with CMS/IRS and on your claims.
