medicaid primary claim denied

patient have just medicaid insurance.claim is 90 days old.can you tell me what will be appropriate delay reason code for submit claim.

Obviously you are not in the U.S. I wonder if you are billing for a state that doesn't allow for offshoring outside the U.S. for Medicaid.

Not to mention we could not possibly give you a delay reason without knowing the reason for the delay.   ???
