Your information does not tell what dx codes are billed in what order for what services. 

This is what I'm saying when I say we don't have enough information to tell you what's wrong.  There is really no way for you to provider ALL information in a post, although I can tell you try  :D.  There are so many components to billing and without having access it's impossible to tell. 

Instead of giving all of the information you are giving, because much is not helpful in determining the billing issue, it would be better if you stated the issue.  So here the issue (I believe) is they are only paying 1 injection.  So did the injections have the correct dx's assigned to them?  You may have to file an appeal.

You had the dxs listed in the original post, the issue is it matters greatly what the dx pointers are on each line.  Just putting the dx codes on the claim is not enough.  The pointers must show the dxs for each service.  Most likely you do not have the dx pointers right.

This should not be allowed to continue!! This person continuously posts patient PHI. Dates of service are PHI, not to mention they are copy/pasting directly from the EMR along with doctors name and signature note dates.. CLEARLY a violation of HIPAA.. Perhaps the information posted would serve the provider well to help them understand the ramifications of outsourcing outside the U.S with these posts to give them an example.

Whether you meant it or not, you now have a problem. I suggest you follow your compliance plan on how to deal with the disclosure. Assuming you have a BA agreement?
