My PM (eCW) is kicking out J1080 Testosterone testosterone cypionate, 1 cc, 200 mg. I am getting conflicting reports about its validity on my online searches.  Is this a valid code or not?

I did find these codes as well... but J1080 wasn't even on the list with CMS.  I am trying to bill BCBS.

Testosterone
cypionate and estradiol cypionate, J1071
enanthate, J3121
undecanoate, J3145

J1080 was deleted on 1/1/2015.
