can anyone guide me .medicaid denied the claim due to specialty/taxonomy code. denied code 145.

Have you tried to contact Medicaid?  It sounds like the provider is billing for something that they are not enrolled to be able to bill.  But you really did not provide much information so it's not easy to say.

CMS 1500 form Box 33 Provider Information
Specialty/Taxonomy: enter code (eg.225100000X)

Specialty/Taxonomy: found on NPI Lookup site
