New vs established

I am working with a provider who was previously employed by a hospital and now started the new practice and billing under new tax ID and new group 2 NPI. Most of his patient has moved to him. Should I bill these patients as new or established?

Face to face trumps Tax ID/Group NPI. If provider has seen patient for face to face services in last 3 years, they are established. There is a decision tree about this in the CPT book, and also readily available on-line.
