Anthem/ Taxonomy

Where is Anthem requiring the taxonomy be on a hcfa 1500?
I have it in boxx 31b but still getting denials

For all our payers, the taxonomy code goes in Box 17a with the qualifier ZZ

Also can be required in 24J in the shaded area.
