Form fee

What do you all do about patients coming in needing forms filled out?  FMLA handicap tag forms and such.  If you charge for these would you care to share what you charge

The only form we charge for filling out is when someone is trying to go onto permanent disability through Social Security. We charge $25 for that. Everything else we do for free. I know of offices that charge anywhere from $10-$50 to fill out forms, depending on which form it is. My husband's ortho office charged $25 for the doc to fill out his short term disability forms each time they needed to be filled out when he was off of work for 3 months after shoulder surgery.
