PEDIATRIC VACCINES

I am new to family billing... How do you bill kid's vaccines?

I have my chart, but when it says bill one unit of 90460 and four units of 90461
do they mean 90460 1 unit and 90461 4 units 

                           or
90460 and 90461  90461  90461  90461

I have read it both ways. Does it depend on the insurance co? 

HELP!!

I believe the standard way is to bill with one line, 90461 with 4 units, however it may vary by insurance carrier.  There is always one carrier that wants it different from everyone else!
