MODIFIER 25 GUIDELINES

If I bill office visit code 99203 with Lab code 80053, or urine analysis 81001.

Do I need to add modifier 25 with 99203 and "QW" with lab codes? I mean do I need to add both modifier together, or just any one modifier will work.
