This may be a dumb question but... The patient is filing their own claim form because we don't file when insurance doesn't cover services. We provide the member with clinical support docs and itemized statement to file a claim for denial so they can start the appeal process. Anyway, the member brought in their BCBS claim form because there's a place for the provider to sign. My two part question is this: Is the provider truly required to sign the member claim form? If so, why? (We never sign claim forms when we/the provider don't file.)
