Hi,
does coding for OP PT have to be changed or have a modifier to exceed the $1500 cap outside of hospital due to medical necessity? or is it adequate to document need in documentation?

Are you referring to a specific carrier? I'm only aware of Medicare's cap and that is $1740.00.
