care Plan Management

On date of discharge can an attending physician/ARNP bill for 99379 or 99380 Care plan oversight?
Do most plans cover date of discharge physician billing? 99315 99316?

Chris:

    Basic concept is we cannot bill two 99 codes on the same DOS for the same Dr. Insurance adjudication system will not accept when we do so.

Here Care plan CPT's indicates, Dr have went thru the progress of the PT thru records and making decisions on integrating new plan of treatment for the PT care.

But when the PT is being discharged from facility, what changes we can show insurance in writing other than his Rx.
