my question may be a little stupid, but here it comes.. i've been checking NCCI table on medicare website for bundled codes, but some insurance carriers pay for these cpt separately. is there a ncci table for every insurance carrier?

Not all insurance carriers post that information. Our local bcbs has a tool on their website that allows us to see how a claim will be processed prior to submitting it, but most don't. It would be nice if they did!

Yes, other than Insurance companies Humana, Cigna, UnitedHC, Aetna they donot have a set guidelines for CCI edits.
In
www.CMS.HHS.GOV
we have NCCI where in this will not be sucessfull for all the insurance companies.
Additionally if insurance company you work on is Medicare HMO they will definetly follow Medicare CCI
