Place of service question

The speech therapist that I bill for usually treats patients at the home of the patient (not a home health agency).  But sometimes she treats patients at a private school for children with special needs.   Do I put address of the school in Box 32 when the patient is treated there?  The school facility is not in her demographics info with the insurance carrier.

The place of service info goes in 24-b. If it is in the patients home you would use "11".  For the school, you could use "33".
Box 34 should show the provider's normal address of his/her business address or office as on record with the appropriate insurers, not the address of where the treatment was done. I have a client that is a speech therapist, and most of her patients are treated in their homes. It should make no difference to the insurers where the treatment was done for this type of treatment.

Sorry....but I disagree with the above quoted information. 

~The Place of Service for HOME is 12, NOT 11.
~The Place of Service for SCHOOL is 03, NOT 33.
~And, YES, it does make a difference to the insurance companies WHERE treatment was done; if it was the HOME, leave block 32 blank; if it was the child's school, you need to put the name and address of the school in 32.
~there is NO block 34 on the CMS-1500 form.

thank you, yes I know 12 is home, 11 is office, 03 is sch.   My question was about box 32, which you answered.   Was wondering though if insurance company needs to have on file that this provider sometimes treats patients at this particular sch.

Just to be clear - the therapist is not affiliated at all with the school.  We file claims for her as an individual priviate speech therapist
