Confused about type of bill to use

Not sure the question.   ub forms are for facility.   Are you a facility?

they are outpatient facility, providing chemical dependency counseling by a clinician

If it is outpatient you will need to bill on a CMS 1500 (HCFA) not a UB04 and if it is in an outpatient office setting the place of service is 11.

Better way to understand is: If you are billing the professional component of the services as your 90806 suggest, it would go on a CMS 1500.  Facility charges go on a UB form.
