Consult Date Vs Dictation

Hello!,
I am billing for a practice who does hospital consults. Most of the times docs dictates notes on next day of consult or may be after few days. Even though docs states the correct consult date in dictation, transcription line is taking the date of dictation as consult date. Medicare always ask for consult notes. So when there is discrepancy of consult date on the notes, we send addendum for correct consult date with docs signature. Until now Medicare was considering the addendums and were processing the claims but recently I spoke with 2 Medicare reps and they both advised that Medicare will not consider addendum and doc will have to redicatate the notes. As per me and my docs knowledge, they cannot redicatate the notes by law.
Any one is aware if doc can redictate the notes? Any help to overcome this issue is appreciated.

it sounds like doc needs to speak with transcriptionist. this is why I love, love, love having electronic medical records. you may have to use his notes from the hospital chart. and no I dont think he can redo, wouldnt that fall under fraud, (doing something just to get paid by carrier)?

sorry, i looked again and is he using a transcriptionist or a speaking software like dragon

Thank goodness I dont' have to worry about that.. My doctor will have to look me in the eye and NOT stare down at his laptop or Ipad.. I opt OUT.

I agree that the problem is with the transcriptionist. Is he using a service? I would tell them they need to get it straight cuz it's costing doc money. There are many transcription services and options out there!
