Ladies, thanks for response. Sorry for delay. I was stuck in storm so backed up with work.

I agree that it is transcription issue. My docs do inpatient visits so they use hospital transcription systems. Docs have not control over transcription company. They even don't know who does transcription for hospitals. We deal with more than 5 hospitals.

Hopefully hospitals will start using EMR soon.

just to throw this out there, a couple of my providers use the dragon speaking software.   we have emr, however, for reports that can be anywhere from 10 pages and up, the dragon software comes in handy.
