We have billed a claim and Medicare has processed it but now we came to know that we have billed a cpt code in error and mcr has process the claim. So now we are trying to submit a corrected claim to medicare with the correct codes.

If you are changing the CPT code, you can fill out a Medicare Reopening Request Form. Here is an example of the form from my area.
