Is anyone having trouble with billing 99391 with 90460 as a bundles service?EG

Hi,
Does anyone can shed some light on this denial. Medicaid is denying both these charges as bundled.
Any response will be greatly appreciated!!

I believe you have to have the 25 modifier on the 99391 (if the patient's notes support that).
