no fault work comp re evals

when billing a 99214 for a chiropractic re eval in NY does anyone have a standard form to send along with billings other than just the doctors notes indacating a mini exam was done some insurances are rejecting notes asking for more info. Any suggestions to where i can get such a exam form. thanks for any help anyone can offer

I'm not aware of any form specifically for sending with the 99214 - re-eval. I've always just sent in the drs office notes when requested. The only time I've seen them not pay is if the drs notes are not good.
Anybody else?
